A flyer with order form from W. R. Riley Dist'g Co., of Kansas City, MO. It measures 9-1/4" x 5-1/2" and is undated, aside from the "191_" shown on the order form. It is identified as "Form 112". This side consists of three panels, one a form for entering names of imbibing friends, the second contains shipping information, the third shows a gallon demijohn and quart bottle of "Riley's Number 10" whiskey. The obverse contains an order blank.
