A price list and order form sent to a Mr. Billie Hayes, of Warrior, AL., from the Distilleries Sales Co. of Kansas City, MO. The complete package contained a cover letter, five inserts and order forms, and a return envelope. The original mailing envelope is dated November, 1911. This item features a price list for various whiskies on one side and an order form for Bohemian beer on the obverse. It measures 5-1/2" x 8-1/2".
