An order form for "Genuine Straight Kentucky Whiskey", from E. J. Reefer, owner of the Green Mountain distillery of Kansas City, MO. One side describes a deal on 5-gallon barrels of whiskey and includes an order form, the other a deal on quart bottles with an order form below. The form measures 8-1/2" x 5-1/2".
