A price list and order form from the Vogt-Applegate Co. of Louisville, KY. One side bears engraved images of an "Old Beechwood" whiskey quart bottle and demijohn and a price list, the obverse bears an order form. It measures 5-1/2" x 8-1/2" and is undated, although the order blank shows 191_.
